Before Seeking Services in Idaho

Moving is often a stressful transition as it is. However, there are steps you can take to ensure that you will still receive the services you need through your move.

1. Notify Social Security about your address change

The first step let the Social Security Administration (SSA) know that you are moving to a new state as soon as possible. You can call them or you can update your SS information on your online profile.

Tell the SSA about the change at the latest by the 10th day of the month after it happens. If you moved sometime in January, you would tell Social Security by February 10th. However, you should notify the SSA as soon as you can about your move.

You can visit the Social Security website here to read more or call them at

+1-800-772-1213.

2. Call Primary Care Providers to check availability in taking new Medicaid clients before arrival

Schedule an appointment to establish care with a Primary Care Provider; this appointment should be 1-2 weeks after your arrival to ensure Medicaid will be in place.

3. Once you arrive in Idaho, enter a Self-Reliance office at Health & Welfare to apply for Medicaid

Visit a Self-Reliance office in person with the necessary documents to apply and get approved for Medicaid. Don't forget to bring proof of assets as well.

If you're not sure where to find an office, you can locate one here on the Idaho Health & Welfare website. Make sure the office you choose offers Self-Reliance Benefits Assistance.

You can call Health & Welfare at 208-334-6700 if you have any questions about the process.

4. After Social Security has switched to Idaho and Medicaid is established, apply for services

The other pages on this website will help you narrow down what specific services you're looking for, or you can select from the options below:

Aged & Disabled (A&D)

Developmental Disabilities (DD)

Traumatic Brain Injury (TBI)

Developmental Disabilities Agency (DDA)

Community-Based Rehabilitation Services (CBRS)

Case Management

Counseling

If you have questions about any applications, feel free and call Malori (208-960-4015) for questions regarding the A&D Waiver or Liz (208-599-6592) for questions regarding the DD Waiver. We are available and happy to help with your transition into care!
