Qualifying for Home and Community Based Services (HCBS)

Participants must meet Medicaid financial eligibility as well as medical disability criteria in order to be eligible for Developmental Disability (DD) services. To meet medical disability criteria, your disability must be diagnosed:


 –  A chronic condition

 –  Before the age of twenty-two (22) 

 –  Caused by impairments such as intellectual disability, cerebral palsy, epilepsy, autism, or another condition found to be closely related to, or similar to one of these impairments that requires similar treatment or services; or be caused by dyslexia resulting from one or more of the above described impairments; and

 –  Substantially limit your ability to function in at least three (3) of the following major life activities;

  • Self-care
  • Receptive and expressive language
  • Learning
  • Mobility
  • Self-direction
  • Capacity of independent living
  • Economic self-sufficiency

 –  Reflect the need for a combination and sequence of special, interdisciplinary, or generic care, treatment or other services

 –  Require services which are of lifelong or extended duration

 –  Require services that must be individually planned and coordinated

Step-by-step process for DD WAIVER RESHAB services

1. Submit an Eligibility Application for Adults with Developmental Disabilities to the Bureau of Developmental Disability Services (BDDS). An application can be found online at the Adult DD Care Management website. 



2. Demonstrate financial eligibility. In order to be eligible, all current assets of the client must not exceed $2000 in value. 

3. Get reviewed by an Independent Assessment Provider (IAP). Provide a current health & physical report that shows a DD diagnosis, psychological evaluation if applicable, and a completed DD Medical Care Form. 



4. Receive an eligibility letter and budget from Medicaid

5. Meet with a Service Coordinator and care team to learn about available services and choose a service mix that fits the budget. A list of Service Coordinators can be found here.


6. Meet with Residential Habilitation agencies to select one that will be a good fit.

7. Create a plan with your Service Coordinator that specifically names the agencies that will provide services. The Service Coordinator will submit the plan to the Medicaid Care Manager, who will review the plan within 14 days of submission. Upon approval, requested services can begin.

8. Choose your focus. The ResHab agency collaborates with the care team to tailor a program entirely around client needs and preferences. Those preferences are protected by Home and Community Based Services (HCBS) laws which empower clients or guardians to decide when, where, and which services are received. 

9. Make changes as needed. Adjustments to the plan can be made throughout the year by contacting the Service Coordinator and submitting an addendum. Changes may involve selecting a different agency, modifying services, or adjusting service hours per week, ensuring flexibility and client-centered care.
